As Minnesota transitions to Community First Services and Supports (CFSS), having access to the right resources can make your journey smoother and more successful. Whether you’re new to home care services or transitioning from Personal Care Assistance (PCA), we’ve compiled a comprehensive list of resources to help you navigate this program effectively.

To clear up confusion regarding “who does what” regarding CFSS, we have created this roadmap. Take a look!

Step A

Your reassessment is completed. Your assessor will give you a list of Consultation Services Providers. If they do not give you this information, please ask them for it. You or your Responsible Party (RP) will make your selection and report it to your assessor.

Step B

Meet with a Consultation Services Provider and choose your CFSS Agency or Budget Model as well as your agency of choice.

Step C

Your Consultation Services Provider sends your new Service Delivery Plan to your county.

Step D

The county approves the Service Delivery Plan and your Service Agreement, then sends a copy to your agency of choice.

Step E

Services with Best Care start when we receive your Service Delivery Plan and your Service Authorization from your County Case Manager. You will also need to complete the orientation with Best Care’s Supervising Professional/QP.

What are the Differences Between the Agency and Budget Models?

Best Care provides both the Agency Model and the Budget Model under CFSS. In either the agency or budget model, the services you are eligible for will remain the same. Both models have these options:

  • A person’s spouse or the parent of a minor may serve as that person’s support worker/caregiver.
  • A person who uses CFSS may serve as the support worker/caregiver for another person who uses CFSS.
  • People may choose to buy goods and services to aid in their independence.
  • The consultation services provider will provide education on the person’s options and support in writing their service delivery plan.

Agency and Budget Model Comparison ChartCFSS Agency Model: You select a provider agency that serves as the caregiver’s employer. In the Agency model, your role and benefits to your caregivers remain the same with Best Care. The provider agency is responsible to recruit, hire, train, supervise, and pay your caregiver/support worker.

CFSS Budget Model: You employ your support workers and have a budget rather than service units. You are responsible for the recruitment, hiring, training, and supervision of your support workers. You also select a financial management services (FMS) provider, from a list which includes Best Care, to help you with payroll. Under this type of employer structure, certain family members are exempt from Social Security and Medicare, and not covered by Unemployment. Talk to your FMS for more information.

Below are some additional frequently asked questions that we have been receiving from clients and recipients of care.

What is CFSS?

Community First Services and Supports. It is replacing PCA (Personal Care Assistance) and CSG (Consumer Support Grant).

Is Best Care a Consultation Services Provider and is this a requirement for receiving CFSS services?

NO, Best Care is not a Consultation Services Provider. YES, it is a requirement. At the time of your reassessment, your County Assessor will provide you with a list of Consultation Services Providers. These providers are also listed on MN DHS website.

If you choose Best Care for CFSS Agency Model, will your caregivers' or employees' wages be lowered?

NO! Your caregivers/employees that are currently working for you with Best Care will not have their wages lowered if you choose the Agency Model.

What services does Best Care offer?

  • PCA Choice (until your reassessment)
  • CFSS Agency Model
  • CFSS Budget Model
  • FMS (Financial Management Services) includes CDCS
  • 245D Basic Waivered Services

Can a client stay with Best Care after their reassessment?

YES! We offer both Agency and Budget Models under CFSS services. You must meet with your chosen Consultation Services Provider first!

Does Best Care offer PTO and Holiday pay to caregivers/employees if the Agency Model is chosen?

YES! Best Care does offer both PTO and Holiday Pay with the Agency Model and PCA Choice.
