What Are 245D Basic Waivered Services in Minnesota?

According to the Minnesota Department of Human Services, 245D Basic Support Services are:

“Services that provide the level of assistance, supervision and care that is necessary to ensure the health and safety of the person.”

Best Care is a licensed provider of 245D basic services in Minnesota. This includes homemaking, respite, adult companion, personal support, and night supervision. For each service, the recipient selects a trusted worker who best suits his or her needs. The recipient maintains control of how his or her worker(s) provide services by scheduling hours and providing them direction on specific day-to-day support needs.

Homemaking Services

Homemaking services help an individual manage general cleaning and household activities. The three homemaker services are: 

  1. Homemaker / Cleaning: This includes light housekeeping tasks and laundry services. 
  2. Homemaker / Home Management: This includes light housekeeping and assistance with laundry, meal prep, shopping for food, clothing and household supplies, simple household repairs, and arranging transportation. 
  3. Homemaker / Assistance with Activities of Daily Living (ADLs): This includes light housekeeping and assistance with walking, bathing, dressing, grooming, eating, and toileting. 

These services are available under the Home and Community Based Waiver programs.

Respite 

This short-term care service is provided when the person’s primary caregiver is absent or needs temporary relief from caregiving duties. Respite provides the level of supervision and care that is necessary to ensure the person’s health and safety and can be provided in the person’s home or at the caregiver’s home. 

This service is available under the Home and Community Based Waiver programs.

Adult Companion

Adult companion services are non-medical services that help a person 18 years or older work toward a therapeutic or community goal through skill building and socialization. Examples include

  • Working on coping skills to manage anxiety
  • Participating in the community to reduce social isolation
  • Providing verbal instructions to help the person complete a task

This service is available under the Alternative Care / Elderly Waiver, Brain Injury Waiver (BI), Community Alternative Care Waiver (CAC), and the Community Access for Disability Inclusion waiver (CADI) Waiver programs.

Personal Supports

This service is provided in the person’s home or community and includes assistance with activities of daily living (ADLs), accessing community services, developing meaningful connections with community members, establishing new relationships and nurturing existing ones, and participating in community activities of the person’s choosing. This service largely focuses on providing opportunities that enables the person to interact with people with and without disabilities to the fullest extent possible. 

This service is available under the Brain Injury Waiver (BI), Community Alternative Care Waiver (CAC), the Community Access for Disability Inclusion waiver (CADI), and the Developmental Disabilities waiver programs.

Night Supervision

This service provides overnight assistance and monitoring by an awake staff in the person’s own home and includes carrying out a person’s positive support programming and transition plans, reinforcing independent living skills training and other skill development supports, and assisting with instrumental activities of daily living (IADLs). 

This service is available under the Brain Injury Waiver (BI), Community Alternative Care Waiver (CAC), the Community Access for Disability Inclusion waiver (CADI), and the Developmental Disabilities waiver programs.
